Visions Unlimited’s Senior Support Program is a peer-driven, home-based support program that provides supportive services to poor, homebound older adults (age 65 plus) who live in high risk neighborhoods in Sacramento County.
The primary goal is to enable older adults to remain in their homes as long as possible and delay costly and undesirable housing alternatives.
Services offered include: advocacy, linkage to community resources, in-home visits, telephone check-ins, transportation, and supplemental food delivery.
